Before submitting your final data for 1095-C processing, you must review and verify your Client Verification Report to ensure the integrity of your employee and dependent data.



Client Verification Report

The Client Verification Report provides an easy-to-review snapshot of the information on file with ACA Track regarding your company’s divisions and health insurance plans. All clients are required to review and approve this report prior to generating your 1095-C forms.

  1. To view your report, select the “Click to View Reports” option.
  2. For specific instructions on reading and interpreting your Client Verification Report, see our Client Verification Report Guide.
  3. If you see any errors, contact your Project Manager to make the necessary changes.
  4. If there are no errors, select the “Click to Verify Report” option.



Generate Your 1095-C Forms

If the information provided in your reports is correct, you’re ready to generate your 1095-C forms.


On the Audit Quality Review tab, select the “Please Generate my 1095 Forms” option.

 

 

 

Before processing your request, you must confirm that your information is complete and all your required dependents have been loaded. Note: if you generate your 1095-C forms and then identify errors, you will need to reload all your data and begin the verification and approval process over again. 

   

Once you select Submit, ACA Track will begin processing your 1095-C files for your review. This process make take some time, so we will email you once the files are available to review. Once they have been created, proceed to the Review & Approve 1095s tab. Note: Your 1095-C files will not be sent until you review and approve those files.
