Most often, an Accepted with Errors response is the result of an inconsistency between the last name and Social Security number listed a 1095 form and the information the IRS has on file. This can occur for a number of reasons, including:

  • The information was entered incorrectly.
  • A maiden name was used when a married name is registered with the SSA (or vice-versa)
  • Multiple (or hyphenated) last names. In cases where an individual has two last names, the first last name listed in the last name field must be the one registered with the SSA.


The attached Accepted with Errors reports lists those employees whose information was flagged by the IRS. ACA Track recommends that you review this report and:

  1. Check your records (HR/Benefits, Payroll data) to verify that      names and SSNs were entered correctly.
  2. Contact the employee to validate the Name and SSN Combination for      the employee(s) and any dependents.
  3. Review Dependent information for any flagged employees. Unfortunately, for privacy reasons, the IRS cannot tell us whether the error was with the employee’s information or one of their dependents, so you should review all covered individuals. 
  4. If you can’t find any mistakes (or if you are unable to contact the employee to verify their information), you should document the steps you have taken as proof of your effort.
